STAR (STOP THINK ACT/ASK REVIEW)

The target is to “think before acting” . It is a basic way of to promote safe acts and safe behavior. It is preventive and not reactive method. 

You can apply as “self-checking” habit of your mind that keeps attention on the task and can help you to pay greater.

It can also apply to your colleagues with STOP only. 

The first step is Stop

Before any decision and actions, avoid impulsive reactions. It can lead to injudicious decisions with unintended consequences. In addition, emotions can play a significant role in decision-making. 

KEEP IN MIND 

1. Skill-based errors are sometimes called errors of the hand (not errors of the head) because the intent of the act is correct – it’s the execution of the act that is incorrect.

2. Skill based errors can be caused by:

a. Inattention (including preoccupation and inadequate mental states)

b. Distractions (both divided and diverted attention)

c. Low alertness (sleepy or fatigued)

d. Time pressure (both urgent tasks and high workload)

The second step is Think

Before taking any action, it is essential to pause and reflect on the situation at hand. It involves cognitive processes that help you to make informed and rational decisions. Take a moment to assess the context, potential consequences, and any available information. 

Some questions :

  • What are you doing? 
  • What about the risks? 
  • What about the less risky method?  
  • How to protect myself and the other?

The third step is Act/Ask : 

Based on the decision-making process done during the « Think » step, it led to a clear and well-informed choice. If some doubts persist, it may be necessary to seek further clarification and information, in this case: Ask 

  • Do it safely and ask if any doubt 

The last step is Review : 

It is an opportunity to check that what you have done were achieved safely and what you expected to happen, happened. It is important to share the lessons learned for the next time if the task must be completed.

  • Did you do the job as safely as possible?   
  • If not, what can you do to make it safer next time?
